Indonesia has gradually extended social health protection to all Indonesian citizens. Achieving universal health coverage (UHC) is mandated under Law No. 40\/2004 of the National Social Security System. However, the implementation was delayed for a decade. In 2014, Indonesia introduced a major health care reform advancing UHC by merging all fragmented health insurance schemes into a single universal scheme for all Indonesian citizens, the Jaminan Kesehatan Nasional (JKN)[1]<\/a><\/sup>. The financial contribution for low-income groups is subsidized by the government, and a mix of public and private providers delivers health service benefits. The JKN is one of the largest single payer schemes in the world. It is managed by the Badan Penyelenggara Jaminan Sosial (BPJS), which also administers Indonesia\u2019s national insurance.<\/span><\/p>\n [\/et_pb_text][ba_gradient_heading title=”Financial protection is a key policy focus ” html_tag=”h3″ primary_color=”#455A64″ secondary_color=”#455A64″ admin_label=”HEADING (50-60 characters)” _builder_version=”4.21.0″ _module_preset=”default” title_font=”||||||||” title_font_size=”24px” title_line_height=”1em” custom_padding=”||10px||false|false” global_colors_info=”{}”][\/ba_gradient_heading][et_pb_text admin_label=”TEXT – BLOCK 1 – ” _builder_version=”4.21.0″ _module_preset=”default” custom_padding=”||1em||false|false” global_colors_info=”{}”]Following the reform to enhance financial risk protection, JKN provided coverage to approximately 248.7 million people, or 90% of the population as of 2022[2]<\/a><\/sup>. Among the covered, health care use significantly increased with the extension to low-income and other socially vulnerable groups. Although health services provided under the JKN are almost free-of-charge, out-of-pocket (OOP) spending on health care remains relatively high at approximately 31.79% percent of current health expenditure (CHE) in 2020[3]<\/a><\/sup>. Evidence suggests that high OOPs are related to the use of private health care facilities and self-medication. There is broad consensus that prevention and early detection could mitigate the severity and progress of disease. The health system made a paradigm shift from curative to preventive-promotive services. It did so by strengthening and incentivizing primary health care services with supporting innovation and technology. This shift was reflected in payment mechanisms linked to performance of primary health care centers that boosted preventive and promotive services such as health examinations and screening.
