Whereas, maternal mortality fell from 173 to 127 deaths per 100,000 live births by 2020. The country still faces a high burden of HIV\/AIDS, tuberculosis and a growing burden of noncommunicable diseases. As a <a href=\"https:\/\/www.parliament.gov.za\/storage\/app\/media\/Pages\/2017\/october\/High_Level_Panel\/Commissioned_reports_for_triple_challenges_of_poverty_unemployment_and_inequality\/Diagnostic_Report_on_Access_to_Quality_Healthcare.pdf\">report on the quality of health care<\/a> in South Africa explains, people in poverty are disproportionately affected (see pages 5-6).<\/p>\n<p>[\/et_pb_text][ba_gradient_heading title=&#8221;South Africa&#8217;s fragmented health system and access&#8221; html_tag=&#8221;h3&#8243; primary_color=&#8221;#455A64&#8243; secondary_color=&#8221;#455A64&#8243; admin_label=&#8221;HEADING (50-60 characters)&#8221; _builder_version=&#8221;4.21.0&#8243; _module_preset=&#8221;default&#8221; title_font=&#8221;||||||||&#8221; title_font_size=&#8221;26px&#8221; title_line_height=&#8221;1em&#8221; custom_padding=&#8221;15px||10px||false|false&#8221; global_colors_info=&#8221;{}&#8221;][\/ba_gradient_heading][et_pb_text admin_label=&#8221;TEXT &#8211; BLOCK 2 &#8221; _builder_version=&#8221;4.21.0&#8243; _module_preset=&#8221;default&#8221; custom_padding=&#8221;||1em||false|false&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<p><span style=\"font-weight: 400;\">Historical inequalities in South Africa led to a two-tiered and significantly fragmented health system based on socioeconomic status, according to a <\/span><a href=\"http:\/\/www.statssa.gov.za\/publications\/P0318\/P03182018.pdf#page=37\"><span style=\"font-weight: 400;\">household survey<\/span><\/a><span style=\"font-weight: 400;\">. This resulted in unequal access to health care, with the public sector serving the <\/span><a href=\"http:\/\/www.statssa.gov.za\/publications\/P0318\/P03182018.pdf#page=37\"><span style=\"font-weight: 400;\">71.5% of the population<\/span><\/a><span style=\"font-weight: 400;\"> who cannot afford private health care.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_text admin_label=&#8221;TEXT &#8211; BLOCK 3&#8243; _builder_version=&#8221;4.21.0&#8243; _module_preset=&#8221;default&#8221; custom_padding=&#8221;||1em||false|false&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">The public health system, primarily funded by taxes, provides free primary health care services for all, according to a <a href=\"https:\/\/www.hst.org.za\/publications\/South%20African%20Health%20Reviews\/2%20Primary%20Health%20Care%20in%20SA%20since%201994%20and%20%20%20Implications%20of%20the%20New%20Vision%20for%20PHC%20Re-engineering%20SAHR%202011.pdf\">report on primary health care<\/a> in South Africa. The report elaborates: hospital-level care is highly subsidized; and public health services, especially primary health care services, face quality challenges due to inadequate funding, staff shortages, drug stockouts, and other limitations.<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">The <a href=\"https:\/\/doi.org\/10.1080\/0376835X.2012.730971\">private sector<\/a> caters to individuals with medium-to-high incomes and serves approximately <a href=\"http:\/\/www.statssa.gov.za\/publications\/P0318\/P03182018.pdf#page=37\">27% of the population<\/a>. The private sector, according to the report cited above, is largely funded by <a href=\"https:\/\/www.hst.org.za\/publications\/South%20African%20Health%20Reviews\/2%20Primary%20Health%20Care%20in%20SA%20since%201994%20and%20%20%20Implications%20of%20the%20New%20Vision%20for%20PHC%20Re-engineering%20SAHR%202011.pdf\">private medical schemes and out-of-pocket spending<\/a>, providing many benefit option packages. The report explains the depth and breadth of coverage in the private sector is based on income and ability to pay and offers quicker access to services than does the public sector.<\/li>\n<\/ul>\n<p>[\/et_pb_text][et_pb_text admin_label=&#8221;TEXT &#8211; BLOCK 4 &#8221; _builder_version=&#8221;4.21.0&#8243; _module_preset=&#8221;default&#8221; custom_padding=&#8221;||1em||false|false&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<p>South Africa has tried to increase the share of domestic health resources from 51.33% to 60.38% in 2021, according to a <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/0376835X.2012.730971\">study reported in Development Southern Africa<\/a>. In 2021, the country\u2019s <a href=\"https:\/\/data.worldbank.org\/indicator\/SP.DYN.LE00.IN?locations=ZA&amp;name_desc=false\">per capita expenditure<\/a> of US$ 583.67 exceeded the average per capita expenditure in middle-income countries. Nevertheless, the authors add, disease burden and financial and nonfinancial resources have not improved. To address them and advance UHC, the <a href=\"https:\/\/www.gov.za\/news\/speeches\/minister-joe-phaahla-president%E2%80%99s-signing-national-health-insurance-bill-15-may-2024\">NHI bill<\/a> was signed into law on 15 May 2024, aiming to introduce a legislative framework directed at creating a single framework for public pooling and strategic purchasing of high quality and accessible health care for all citizens regardless of socioeconomic status, based on equity and social solidarity.<\/p>\n<p>[\/et_pb_text][et_pb_text admin_label=&#8221;TEXT &#8211; BLOCK 5&#8243; _builder_version=&#8221;4.21.0&#8243; _module_preset=&#8221;default&#8221; custom_padding=&#8221;||1em||false|false&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<p>NHI will operate as a <a href=\"https:\/\/www.medicalschemes.co.za\/cms-annual-report-2021-22\/\">strategic purchaser of personal health services<\/a> for the population, with contributions from employers and employees that will be pooled.
