It highlights how both countries shifted from external aid to national ownership in financing and policymaking, offering key lessons for effective donor transitions.<\/strong><\/p>\n<p data-start=\"114\" data-end=\"700\">The transition from donor-supported health programs to <strong data-start=\"169\" data-end=\"208\">sustainable national health systems<\/strong> presents both opportunities and challenges for governments worldwide. This study examines the experiences of <strong data-start=\"318\" data-end=\"339\">China and Georgia<\/strong>, two countries that successfully integrated donor-supported health initiatives into their domestic systems, ensuring <strong data-start=\"457\" data-end=\"508\">long-term sustainability and national ownership<\/strong>. The role of <strong data-start=\"522\" data-end=\"560\">external technical assistance (TA)<\/strong> in this transition has been instrumental, providing strategic guidance, capacity building, and policy advocacy to facilitate the process.<\/p>\n<p data-start=\"702\" data-end=\"1376\">A key success factor in both countries was <strong data-start=\"745\" data-end=\"789\">talent cultivation and capacity building<\/strong>, where external technical assistance played a significant role in strengthening human resources in healthcare management. In <strong data-start=\"915\" data-end=\"924\">China<\/strong>, donor-funded training programs helped develop <strong data-start=\"972\" data-end=\"1031\">a skilled workforce of health managers and policymakers<\/strong>, many of whom later assumed leadership positions in national health institutions. Similarly, in <strong data-start=\"1128\" data-end=\"1139\">Georgia<\/strong>, technical assistance supported <strong data-start=\"1172\" data-end=\"1254\">comprehensive training programs, knowledge-sharing forums, and study exchanges<\/strong>, ensuring the development of national expertise that could sustain critical health initiatives beyond donor engagement.<\/p>\n<p data-start=\"1378\" data-end=\"1982\">Another major contributor to the success of the transition was the establishment of <strong data-start=\"1462\" data-end=\"1526\">long-term partnerships between external and domestic experts<\/strong>. Unlike short-term interventions, <strong data-start=\"1561\" data-end=\"1616\">China and Georgia fostered sustained collaborations<\/strong>, ensuring that knowledge transfer became embedded in national governance structures. In China, external technical expertise was seamlessly integrated into <strong data-start=\"1772\" data-end=\"1823\">subnational and national policymaking processes<\/strong>, while in Georgia, donor support enabled the establishment of <strong data-start=\"1886\" data-end=\"1916\">joint research initiatives<\/strong>, strengthening evidence-based decision-making in health policy.<\/p>\n<p data-start=\"1984\" data-end=\"2642\">The role of <strong data-start=\"1996\" data-end=\"2030\">evidence-based policy advocacy<\/strong> was also a critical factor in ensuring <strong data-start=\"2070\" data-end=\"2100\">effective donor transition<\/strong>. External technical assistance helped generate <strong data-start=\"2148\" data-end=\"2172\">data-driven insights<\/strong>, which were instrumental in shaping <strong data-start=\"2209\" data-end=\"2238\">key health policy reforms<\/strong>. In China, international donors such as the <strong data-start=\"2283\" data-end=\"2358\">World Bank and the UK\u2019s Department for International Development (DFID)<\/strong> played a crucial role in designing <strong data-start=\"2394\" data-end=\"2428\">rural health financing reforms<\/strong>. In Georgia, external support facilitated <strong data-start=\"2471\" data-end=\"2526\">empirical studies on drug policy and harm reduction<\/strong>, leading to the <strong data-start=\"2543\" data-end=\"2598\">institutionalization of opioid substitution therapy<\/strong> within the national healthcare framework.<\/p>\n<ul>\n<li><strong>Insufficient Domestic Funding for Capacity Building<\/strong>\n<ul>\n<li>\u00a0After the donor exit, budget constraints hindered sustained investment in health workforce training and program administration.<\/li>\n<li>Rural and underfunded regions faced greater financial barriers in continuing donor-supported programs.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Dependency on External Implementation Partners<\/strong>\n<ul>\n<li>In some cases, domestic institutions were slow to take full ownership, leading to gaps in project management and service delivery<\/li>\n<\/ul>\n<\/li>\n<li><strong>Variability in Governance Structures<\/strong>\n<ul>\n<li>The integration of donor-funded programs into national health policies varied, leading to inconsistencies in implementation across different regions.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p data-start=\"3378\" data-end=\"3979\">To ensure <strong data-start=\"3388\" data-end=\"3420\">successful donor transitions<\/strong>, several key enablers were identified. The <strong data-start=\"3464\" data-end=\"3517\">institutional integration of technical assistance<\/strong> within public health governance structures allowed for <strong data-start=\"3573\" data-end=\"3635\">smoother knowledge transfer and long-term policy alignment<\/strong>. Strong and <strong data-start=\"3648\" data-end=\"3713\">sustained engagement between donors and recipient governments<\/strong> ensured that policy changes were <strong data-start=\"3747\" data-end=\"3804\">not only implemented but also maintained and expanded<\/strong>. Additionally, strengthening <strong data-start=\"3834\" data-end=\"3904\">local research capacity and promoting evidence-driven policymaking<\/strong> proved essential in maintaining an adaptive and resilient health system.<\/p>\n<p data-start=\"3981\" data-end=\"4547\" data-is-last-node=\"\">The experiences of <strong data-start=\"4000\" data-end=\"4021\">China and Georgia<\/strong> offer valuable lessons for countries navigating similar transitions. Their success underscores the <strong data-start=\"4121\" data-end=\"4287\">importance of investing in local capacity building, fostering long-term partnerships, and ensuring that policy reforms are anchored in data-driven decision-making<\/strong>.
